- Removed mention of primary/secondary adapters in some error

screens since multi-initiator RAID is not supported and the
messages will just cause confusion.
- iprconfig: During disk hotplug, wait for sd devices to show
up. Fixes errors getting logged by iprconfig during hotplug.
- iprconfig: Fix cancel path on concurrent add/remove of disks
- Don't display current bus width and speed for SAS disks
- Fix scoping bug caught by gcc 4.0.
- Stop iprupdate from continually logging errors for adapters with
backlevel adapter firmware.

Added support for non-interactive RAID array creation and deletion through iprconfig. Cleaned up format menus so that only the proper devices show up. Cleaned up uevent handling. Improve iprinit polling mode to do only initialize new or changed devices.

Fix to properly enable init.d scripts when the rpm is installed. Fix memory leak in code download path. Increase size of page 0 inquiry buffer so that extended vpd is displayed. Decrease write buffer timeout to 2 minutes.

Some screen driver fixes. Add additional status messages. Don't allow disks under a JBOD adapter to be candidates for format to 522. Don't allow devices that are being formatted to be candidates for concurrent remove. Cleanup the device states some more.
